ALVIN ISD HAS CHANGED ONLINE SYSTEMS FOR THE 2018/2019 SCHOOL YEAR
**Parents AND students MUST create an account in order to complete forms**
**FOLLOW STEPS BELOW TO COMPLETE PROCESS OR BY CLICKING HERE**
- The student will begin the process by texting their school's Quick Account Code to 69274
- Manvel High School students use code: S40751
- Manvel Junior High students use code: S40755
- Harby Junior High students use code: S40754
- Shadow Creek High School students use code: S40757
- Nolan Ryan Junior High students use code: S40756
- Rodeo Palms Junior High students use code: S40758
- McNair Junior High students use code: S40760
- Alvin High School students use code: S40750
- Alvin Junior High students use code: S40752
- Fairview Junior High students use code: S40753
- Assets Academy students use code: S40759
- Student will be prompted to enter their information including a parent/guardian name and email
- Student will complete all forms as prompted and electronically sign (10 forms)
- Parent/Guardian will then receieve email prompting to register and "link" student to their account
- Parent/Guardian will be prompted to review forms completed by student
- Parent/Guardian will review information in forms for accuracy, fill in their portion and then electronically sign (10 forms)
- Parents/Guardians are responsible for printing off the medical history and Doctor physical exam pages
- Parent/Guardian have the option to either upload physical exam form or turn it in to the MHS Athletic Training Room (H118)
(REMEMBER all physicals MUST be completed AFTER 4/15/18 to be acceptable for 2018/2019 school year)
