    Nurse's Notes and Reminders:

    *Please inform the school nurse of any medical conditions that might affect your student during the school day. (Asthma, ADHD/ADD, Life Threatening Allergies, Physical Disabilities, Surgery, etc.)                                     


    Texas State Law requires immunizations to be current for admission to school. Send a copy to the clinic anytime your child receives them. Please view the link page on this website for immunization requirements.


    Medication Reminder

    All medication must be brought to and picked up at the clinic by a parent or guardian. Students may NOT carry medication to and from school. Prescription medication must be in its original container with the dosage information present on the packaging. Over the counter medication may be kept in the clinic for 10 days with a note from the parent, if medication is needed longer a doctor's note will be required.


    Change of Clothes:

    At times little ones do not make it to the restroom in time, & any student can spill lunch or have a bloody nose. Please place a change of clothes in a plastic bag with your child's name on it in his/her backpack at all times. Clothing donations are always welcomed!



    Please make sure your students immunizations are up to date and that all required forms for allergies, medical conditions, and medication administration have been turned into the nurse before the first day of school. No medication will be accepted until ALL required paperwork, including Action Plans completed by the physician have been completed and turned into the clinic.
