This is a signature form that states each athlete and their parent/guardian acknowledge and understand the rules for participation in AISD athletics.

This is the form you will need to take with you to your physician of choice to have filled out during your physical.  Each athlete must have one on file in order to participate in athelics.

This is the form Alvin ISD has adopted across the district to serve as the code of conduct all athletes must adhere to.   Each athlete is expected to have a signed copy on file with me.
