Allergy
  • Students are not permitted to carry or self-administer epi-pens at school without proper physician and parental paperwork on file. See below for the required form(s).

     

    All epi-pens brought to the clinic need to be in their ORIGINAL box with the pharmacy label including the student's name, date of birth, and expiration date. 

     

    All students that require emergency allergy medication while at school will need to have the following 2 forms completed by their physician and returned to the nurse's clinic each school year.

    Allergy/Anaphylaxis Action Plan (requires doctor and parent signatures)

    Medication adminstration Form (gives parent permission for administration)

     

    If a student has before or after school extracurricular activities, it is suggested that the student self carry their emergency allergy medications. 

    This form is required to be on file for students who self carry and requires a doctor signature.
