    Students are not permitted to carry or self-administer epi-pens at school without physician and parental permission.


    All epi-pens brought to the clinic need to be in their ORIGINAL box with the pharmacy label including the student's name, date of birth, and expiration date. 


    All students diagnosed with an allergy will need to have the following 2 forms completed by their physician and returned to the nurse's clinic each school year.

    Self-Administration of Anaphylactic Meds


    The Medication Administration Form will also need to be completed by the parent/guardian for each allergy medication.
