     Forms That Alvin ISD Requires Each Athlete To Have On File
     
    Physical Form- Every athlete MUST have a current physical on file to participate in Athletics as well as any sport. Please turn in to Coach Davis (for boys) and Coach Nice (for girls)
     
    AISD Athletic Code of Conduct - Can be turned in any time before the last day of school.

    Athletic Forms Parents May Need To Utilize During The School Year
     
     All cross country, and intramural soccer participants must have this form completed and on file before they are allowed to participate.
     
     
    Parents, this form must be completed 24hrs prior to the scheduled competition if you plan on transporting your child from the competition site. If this form is not on file, the player will be required to ride the bus back to the school with the team after the game.
