Applicants who successfully complete the physical agility and written tests will be given a Personal History Statement (PHS) to be completed and returned to the Alvin ISD Police Department.
The following documents (if applicable) must be submitted with the completed PHS, or upon request from the background investigator. Original or certified copies of the documents must be submitted for verification. Original and certified copies will be returned to the applicant at the conclusion of the PHS evaluation or background investigation. Exceptional circumstances for document submission delays may be considered on a case by case basis.

Military Discharge Papers (DD214) from the National Personnel Records Center. An UNDELETED DD214 is required and may be obtained by submitting a Request Pertaining to Military Records (SF180) to the applicable military branch custodian indicated on the SF180, or by submitting an online request to the National Personnel Records Center (NPRC) by using eVetRecs at http://www.archives.gov/veterans/military-service-records/ The SF180 may be downloaded here (SF180) or provided by the Alvin Independent School District Police Department Training Coordinator.
13. Credit Report
A current credit report must be provided from one of the three major credit reporting bureaus; TransUnion, Equifax, Experian. A credit report must be printed with a date that is less than 30 days from the date in which your PHS is submitted. It is recommended that applicants request a copy of their credit report once in the background investigation process.
